Power of Attorney
Service Description
A power of attorney must always be signed in front of a notary public. A financial power of attorney must be signed by the principal, and the signature must be acknowledged before a notary public. A health care power of attorney must be signed and dated in the presence of two witnesses. The document must also be notarized by a notary public.

Otis will call you to confirm the details of individual appointment, including location Please allow a 20-minute window before and after your scheduled appointment for the service provider to arrive All signatories MUST be able to communicate directly with the Notary. Translators are not permitted for notary services All signatories/applicants MUST be present and have a valid Driver’s License or approved Government issued photo ID Please have all documents completed not NOT signed before Otis arrives You may cancel or reschedule up to 1 hour prior to your appointment PAYMENT IN FULL IS DUE AT THE TIME OF SERVICE
