Consent to Travel (Minor)
Service Description
The Consent to Travel is required by the TSA (Transportation Security Administration) for any minor traveling alone or escorted by an adult who is not their parent or legal guardian. The Consent to Travel form is used to show the consent of the child’s parents or legal guardians to travel, confirming the child is not being abducted or taken without permission.

Otis will call you to confirm the details of individual appointment, including location Please allow a 20-minute window before and after your scheduled appointment for the service provider to arrive All signatories MUST be able to communicate directly with the Notary. Translators are not permitted for notary services All signatories/applicants MUST be present and have a valid Driver’s License or approved Government issued photo ID Please have all documents completed not NOT signed before Otis arrives You may cancel or reschedule up to 1 hour prior to your appointment PAYMENT IN FULL IS DUE AT THE TIME OF SERVICE
